HIATAL HERNIA: The diaphragm is the large, flat muscle at the base of the lungs that separates the chest cavity from the abdominal cavity. The esophagus passes through a small opening in the diaphragm called the diaphragmatic hiatus. Normally, the LES at the bottom of the esophagus is aligned with this opening in the diaphragm (the hiatus). The rest of the esophagus is located above the diaphragm in the chest cavity, and the stomach is located below the diaphragm in the abdominal cavity.
If you are among the 25 percent of people over age 50 who have a hiatal hernia, your LES and a small portion of your stomach have slipped through the opening in the diaphragm and are now protruding into your chest cavity. The resulting separation between the LES and the diaphragm weakens the barrier against reflux of acid from the stomach into the esophagus, increasing the risk of GERD and erosive esophagitis (damage to the lining of the esophagus). Other risk factors for a hiatal hernia include being overweight and lifting heavy objects.

BARRETT’S ESOPHAGUS is a condition that results from the abnormal healing of erosive esophagitis. Columnar cells, the type found in the intestines and stomach, replace squamous cells, which usually line the esophagus, most often because of repeated exposure to gastric acid. This process is called intestinal metaplasia. Men develop Barrett’s esophagus twice as often as women, with whites affected more frequently than men of any other race. The average age at diagnosis is 55.
The main reason for concern about Barrett’s is that there is a tendency for cancer of the esophagus to develop. This cancer has increased by more than 600 percent since the 1970s. It has the highest increasing incidence rate (3 to 6 persons per 100,000) of any cancer in the United States, and it remains highly lethal, with a survival rate of less than 15 percent, and treatments are not always successful. NOTE: I had stage 4 Barrett’s Esophagus without cancer. The head of Gastroenterology at UCLA, L.A., had never seen this before and took me in, treating the lining of my esophagus in the hope of finding out why.
